Identity + NPI + state + per-measure rates and footnotes. **NPI IS PRESENT** (100% coverage) — the only Care Compare dataset that publishes NPI directly on the agency-level table.

Identity backbone for ASC CCNs (POS QIES coverage incomplete). Deterministic NPI bridge (no NPPES fuzzy match needed) — match-confidence improvement vs other Care Compare datasets. Drives the brand-hub `/care-compare/asc` module. ASC-12 (7-day risk-standardized hospital visit rate) is the headline quality measure.
What this source does NOT mean
ASCs do NOT have a CMS-published overall star rating (unlike NH / HH / Dialysis). Quality is per-measure (ASC-1 through ASC-12); Fonteum does not manufacture composite scores. Suppression for <25 cases applies.

Every field below has a `display_allowed` flag in the §94 provenance schema. Write-locked fields are captured to provenance for audit but never rendered on profile pages.
